The Client Services Manager (CSM), operating as part of the Client Services Team, ensures the delivery of high quality services for clients of Lakeview Supported Living Services (LSLS). With direction and support from the Client Services Director (CSD) and Client Services Coordinator (CSC), the CSM identifies, develops, coordinates, monitors and participates in the residential and community services for clients on an assigned caseload. The responsibilities of the Client Services Manager include, but are not limited to:

  • Developing and maintaining a strong rapport with each client on an assigned caseload.
  • Working a minimum number of direct support hours with clients on an assigned caseload (daytime, evening, weekend or overnight).
  • Developing, implementing, monitoring, reviewing and updating personalized Support Plans for clients on an assigned caseload (with direction and assistance from the CSD and CSC)
  • Maintaining an expert level understanding of the Support Plans for clients on an assigned caseload.
  • Working with the CSC to coordinate and maintain Client and Support Staff schedules for an assigned caseload.
  • Attending internal and external team meetings for clients on an assigned caseload.
  • Collaborating and communicating professionally with internal and external teams and guardians for clients on an assigned caseload.
  • Acting as the primary point of contact for internal and external teams and guardians for clients on an assigned caseload.
  • Facilitating Meet and Greets (introductions) between clients and potential Support Staff for an assigned caseload.
  • Delegating responsibilities and supervising the work of the Support Staff working with clients on an assigned caseload.
  • Providing initial and ongoing training for both new and existing Support Staff per the Support Plans of an assigned caseload.
  • Working with the CSC to track and monitor timekeeping records, tasks, behaviors, wellness questions and shift notes for clients on an assigned caseload.
  • Evaluating the performance of the Support Staff working with clients on an assigned caseload through supervisory visits.
  • Understanding the high level support needs of all clients within the LSLS Network.
  • Answering phone calls and responding in person as the On-Call Manager for the entire network of clients during scheduled/assigned time frames.
  • Participating in the Interview, Training and Development, Performance Review and Termination processes of the agency.
  • Filling in on open or available shifts with all clients in the LSLS Network as needed.

Minimum Qualifications:

  • Associates Degree OR Bachelors Degree
  • At least 2 years of Health Care, Social Services or Direct Support experience or related college level courses.
  • Ability to successfully pass a Caregiver Background Check.
  • A valid driver's license with personal transportation that is reliable and insured.
  • A smartphone with suitable voice and data plan for daily calls, email and internet usage.
